Our pathways focus on patient populations and not individuals. Do people make patient specific pathways? Wouldn't that just be an ITP?

Vicki, yes. You can look at CP from the perspective of a population as defined as "all our depressed pts" or all our pts at a certain level of care. You can also look at it as the pathway to recovery you and an individual client develop together as their treatment/recovery/person centered plan. Here we are focusing on the "population" level CP that teams develop and execute.

does anyone have any experience with a collaborative care model between PCP and child psychiatry? We have implemented both adult and child psychiatry Col Care model and the child psychiatry segment is not doing as well as the adult collaboration
