We have several federal grants in a rural area so we are required to have specialty teams. It’s been tricky ensuring clients aren’t disrupted in their care from one funding stream to the next. It can be frustrating for employees to serve clients when it’s dependent on what funders want and need.

Kim, preventing staff and clients from feeling the "seams" in the system like what their payer will pay for or not falls on the team to make sure the person centered plan is best aligned with what can be paid for and what is needed by the person to be healthy. No small task b/c if one client can get more services b/c they have better ins that will really frustrate things..
